How to Register

  1. All ladies are invited to register, mothers and daughters ages 12 and up, younger by parental discretion.

  2. Download and print the COMMIT Registration FormPDF Format and the COMMIT Information SheetPDF Format.

  3. With your parents, prayerfully fill out the registration form and information sheet.

  4. Mail or fax the completed registration form to:
